Naxxramas is the final 40-player raid of Classic Era and the most challenging vanilla content. The instance floats above Eastern Plaguelands and contains fifteen bosses split across four wings (Spider, Plague, Military, Construct) plus the Frostwyrm Lair (Sapphiron, Kel'Thuzad). Each wing has a unique theme and loot table; wing-end bosses drop the Tier 3 set tokens that combine with Cured Rugged Hide and other reagents into the BiS gear of vanilla.

The encounter scripts are the most demanding in the game. Razuvious requires Mind Control rotation by priests. Four Horsemen requires precise positioning of two tank groups across the room with debuff swap rotation. Heigan's Dance is a movement check killing players on positioning errors. Sapphiron's Frost Resistance gear requirement forces the entire raid to maintain a 100+ frost resist budget. Kel'Thuzad's three-phase encounter ends Classic Era progression.
